Onychocryptosis (also known as an “Ingrown nail,” or “Unguis incarnatus”) is a common form of nail disease. It is an often painful condition in which the nail grows so that it cuts into one or both sides of the nail bed. While ingrown nails can occur in both the nails of the hand and feet, they occur most commonly with the toenails.

Causes

The main cause for onychocryptosis or “ingrown nail” is improper footwear including shoes with inadequate toe-box room and tight stockings that apply top and or side pressures; next is the damp wet atmosphere toes are subjected to all day in enclosed shoes, softening the nail-plate and swelling the epidermis keratin, which eventually increases the convex arch permanently; next is genetics; and last are trauma and disease.

Improper cutting of any nail may cause the nail to cut into the side-fold skin from growth and impact, whether or not the nail is “ingrown” (onychocryptosis). The nail bends inwards or upwards depending on the angle with which it has been cut. Looking from the perspective of the owner, when cutting the nail, if the cutting tool, such as scissors, are in an attitude where the lower blade is closer to the toe than the upper blade then that will cause the toenail to start growing from its base upwards.

Vice versa, when the lower blade is farther than the upper blade, the toenail will turn inwards. The process is visible along the nail as it grows, appearing as a warp that advances towards the end of the nail. The upper corners turn more easily than the center end of the nail. As people cut their nails by holding the tool always in the same angle, they induce these conditions by accident, while as the nail turns closer to the skin, it becomes harder to fit the lower blade in the right attitude under the nail. When cutting a nail, it is not just the right angle that is important, but also how short it is cut. A shorter cut will bend the nail more, unless the cut is even on both top and bottom of the nail.

Causes include:

  1. Bad maintenance, including cutting the nail too short, rounded off at the tip or peeled off at the edges instead of being cut straight across
  2. Ill-fitting shoes, as those that are too narrow or too short can cause bunching of the toes in the developmental stages of the foot (frequently in those under 21), causing the nail to curl and dig into the skin
  3. Trauma to the nail plate or toe, which can occur by stubbing the toenail, dropping things on the toe or going through the end of the shoes (as during sports or other vigorous activity), can cause the flesh to become injured and the nail to grow irregularly and press into the flesh
  4. Predisposition, such as abnormally shaped nail beds, nail deformities caused by diseases, or a genetic susceptibility to nail problems like ingrowth

The Laparoscopic Adjustable Gastric Banding Procedure (also know as lap band surgery) is performed under anesthesia in a hospital operating room. During surgery, three to five one-half inch to two inch incisions are made in the upper abdomen. Barring any complications, the procedure usually takes about 50 minutes to one hour to perform.

All patients undergoing this procedure need to be aware of the possibility of complications such as adhesions from previous surgeries which may make a laparoscopic procedure impossible. In this case, the surgeon will convert to an open procedure with a five inch incision and longer recovery period.

Many surgeons allow patients to return home the same day; however, other surgeons require patients to stay overnight before they are released from the hospital. If this occurs they are usually released about 24 hours after surgery. Although most lap band patients feel quite well after two or three weeks, full recovery usually takes approximately six to eight weeks. When you are ready to leave the hospital, you may receive a visit from the hospital dietitian who will go over the required diet for Lap-Band patients

During the lap band surgery a smaller stomach pouch is created in the upper portion of the stomach. Normally, this pouch will only hold approximately ½ to 1 ounce of food. This pouch connects to the rest of your stomach through an outlet called a stoma. You will feel full when your upper stomach is full and as a result you will be able to control your eating habits and over time, with the right eating habits, you will start to lose lots of weight. And it’s natural weight loss so once you lose the pounds they won’t just fly back on.

In essence, the lap band surgery is a procedure aimed to promote weight loss for people who are severely struggling to cope with obesity. Here’s how it works in greater detail:

A ‘gastric band’ with a small balloon inside is attached around the top portion of your stomach, forming a smaller ‘gastric pouch’. The placement of the band creates a small pouch at the top of the stomach that holds up to approximately 30 ml, which works out to about 1/8 cup. You are inclined to eat less because the pouch holds less food than the whole stomach. As the upper part of the stomach registers itself as being full, it sends a message to the brain saying that the entire stomach is full. This is the main principle behind the lap band procedure; this tightening of the stomach feeling or sensation helps the person to:

1. be hungry less often
2. to eat smaller portions
3. and lose weight over time.

This pouch is filled with a saline solution which is injected into the band via a port. Via the port, the surgeon has control over the amount of saline solution that is inserted into the band. When adjusting the lap band (same as gastric band) a specialized needle must be used as to avoid causing any damage to the port or the port membrane. When fluid is injected, the lap band expands and places pressure around the outside of the stomach. This causes a decrease in the size of the passage between the pouch and the lower stomach; restricting the movement of food.

After the procedure, the next stage is the post lap band surgery follow-up adjustments. During this stage, the patient will need to visit their doctor every few weeks in order to receive their saline injections. Over time, the band is filled to the point where the patient feels they’ve hit a ’sweet spot’ where optimal restriction has been achieved. Drinking shows little effect on stroke outcome

While some research has suggested that moderate drinking may lower a person’s odds of suffering a stroke, a new study finds that it may have little long-term impact on stroke risk or stroke severity.

The findings, reported in the journal Stroke, come from a more than two-decade follow-up of nearly 22,000 U.S. male doctors. Researchers found that overall, there was no strong association between the men’s drinking habits and their odds of suffering a stroke.

Nor was there a clear connection between alcohol intake and the severity of disability following a stroke.

Some past studies, though not all, have suggested that light-to-moderate drinking may be protective against stroke — as it appears to be against heart disease. But the current study, which followed participants for an average of 22 years, was longer term than those earlier studies, the researchers point out.

In addition, a number of other studies have found that the protective effect of moderate drinking is generally weak and fades with longer-term follow-up, noted senior researcher Dr. Tobias Kurth, of the French national health institute INSERM, in Paris, and Brigham and Women’s Hospital in Boston.
